Esperanza Cantu (2019 cohort) helped develop a Community Information Exchange in Detroit as part of her fellowship project, and has since grown it into Connect4Care, a network connecting more than 40 community partners. By enabling organizations to coordinate information and referrals, the CIE creates more seamless pathways to healthcare, food, transportation, and other essential services for people navigating complex needs.
